Center “Nos Perseveransia” has been operational since 2013 as part of the treatment of Mental Health Caribbean on Bonaire, where a diversity of care products is offered. These include: temporary residential care focused on learning household and social skills, detox, counseling in one’s own environment (outpatient care), daily activities and back-to-work guidance. All towards reaching the highest attainable form of independence.
For whom?
Sentro Nos Perseveransia offers guidance to adults with:
– addiction problems (with light mental disability) and / or
– psychiatric problems (with light mental disability)
Guidance
The guidance offered is aimed at social recovery, empowerment and experiential expertise, so that participants, despite temporary or permanent limitations, can function satisfactorily in the environment of their choice.
What do we do?
Participants are guided in creating an individual program. This program is aimed on the one hand at housekeeping, self-care and structure building, and on the other hand at (creative) activities for during the day, relaxation or labor guidance.
The program is upheld by means of a support plan, a treatment plan and a signaling plan that are designed together with the participant and his/ her system. Housing, finances, social functioning, psychological functioning are some of the topics that are discussed.
The center aims to teach practical, communication and social skills. Budgeting, day spending, labor guidance, and relapse prevention will be included as standard in the (residential) guidance. Participants flow out to a place (home) in society that is suitable for them, with a solid foundation and further support.
Why?
Psychiatric and/ or addiction problems are interwoven with problems in multiple areas of life. Due to skills shortages, marginalization or a combination of both, many participants are insufficiently able to sustain themselves in their living environment. To lead an independent, stable life a good living situation is of great importance. A large part of the participants can only be reached for care if this vital necessity is met. To achieve this, cooperation with external partners is absolutely necessary.
Social recovery starts as soon as a support plan has been set out and the participant is able to work on recovery together with the housing counselor in a development-oriented way. The counselor offers a defined system of activities, which helps the participant to develop the skills needed to function as independently as possible or in a protected environment. A case manager is appointed as internal and external factors must be properly managed.
Center “Nos Perseveransia” is based on the autonomy of the participant and is aimed at activating the life opportunities of a participant. The care needs of the participants and their support system in different phases of the care process are met. Depending on the possibilities of a participant, the (residential) supervision varies from an extremely minimal to very intensive care, from preventive to protected care.
Registration
Registration for guidance at the “Nos Perseveransia” center always goes through the F-ACT team for adult care.
